Scheduling Office

Our scheduling office is open Monday through Friday from 8:00 a.m. to 5:00 p.m. To help us schedule the right amount of time for your visit, please let us know:

  • Which provider you want to see
  • Why you are being seen

If you need to be seen on a same day basis, please call first. This will allow our clinic staff to make the best arrangements for you and have your medical record available for the doctor. If you need to cancel an office visit, please notify us as soon as possible.

Walk-in visits are available Monday through Friday from 8:30 a.m. – 10:00 a.m. and from 1:00 p.m.-2:30 p.m. You do not need an appointment to be seen in our Urgent Care which is open:

  • Monday, Wednesday, Friday 5:00 p.m. - 8:00 p.m.
  • Saturday 11:00 a.m. – 3:30 p.m.

Telephone Calls
Our main telephone number is (612) 302-8200. If you need to schedule an appointment, select option one. If you need to speak with the doctor or the nurse, select option two. Please be ready to give information such as:

  • Name of patient
  • Date of birth
  • Temperature
  • Length of illness
  • Symptoms

A nurse will return your call as soon as possible, usually within two to three hours. If you need to speak to a nurse right away, please tell the person taking your call or press zero.

Child Care
We offer child care services for our patients while they are seeing the doctor Monday through Friday from 8:30 a.m. – 5:00 p.m. Please ask about this service when scheduling your office visit.

Test Results
If your doctor orders lab tests, you will be called with the results if they are not available on the same day of your appointment. Some tests must be sent to another lab. If you have not received your test results after two weeks, please call the clinic and ask to speak with a doctor or nurse.

Prescription Refills

Please have your prescriptions refilled at your office visits. If you require refills before your visit:

  • Call your pharmacist
  • Allow 24-48 hours for refills

If the pharmacist cannot refill your prescription, call the clinic during regular business hours.

Referrals

If other specialty care is required, your family doctor will:

  • Refer you to another specialist, if it is medically appropriate
  • Provide the specialist with important information concerning your care

A written referral will be completed for you by the patient representative. This is very important if you have insurance from UCare Minnesota, BlueCross & Blue Shield,
Preferred One, or Medica Premier.

Medical Records

A valid release of information form is required by our Medical Records Department before records can be copied. Please allow one week for copying, if possible.

Financial Policy

If you have insurance:

  • Please bring your insurace card with you to each appointment.
  • Copayments are due at the time of service

If you do not have insurance:


  • Please call our patient advocate at 612-302-8201 to discuss options for yourself and your family.
  • You will be expected to pay a $50.00 partial payment at the time of your visit and will be billed for the balance.
  • All accounts with a balance due will receive a monthly statement, a 20% discount will be given for every bill paid in full on the day of your visit.

Medical Emergencies/Hospital

For a life threatening emergency:

  • CALL 911 or go to the nearest emergency room
  • Contact your clinic doctor as soon as possible

The emergency room entrance at North Memorial Medical Center is located on Lowry Avenue in Robbinsdale. Please contact your clinic before going to the emergency room for any emergencies that are not life threatening.

After Hours Care
For all other emergencies, you can reach one of our doctors 24 hours a day by calling the clinic number. When the clinic is closed, your call will be answered by our answering service. The "on call" doctor will be contacted to advise you.

Interpreters
Interpreters are avaliable at our clinic for Spanish, French, Swahilli, Russian and Hmong languages.

Special Services

North Memorial offers social service assistance and patient education. Your provider can also help arrange psychiatric care.
