Referring a Patient to University of Minnesota Physicians
Our relationship with you, your patients, and your office staff is important to us. Following our evaluation, we will send a detailed report of our findings and recommendations to assist you in providing continued care for your patient.
How to Refer a Patient
For your convenience, we have two options for referring patients to the Memory Clinic for evaluation.
1. Call 612-273-8720 to schedule an appointment
2. Complete the Request for Consultation (PDF) form and fax to 612-273-8727
The following information is needed to make the appointment:
- Patient Information (name, address, birthdate, phone number)
- Referring Physician Information (name, clinic, address, phone number)
- Reason for the Referral (Has the patient been diagnosed? What would you like the consultant to answer?)
- Preferred Method of Communication (phone, fax, or letter)
Prior to the Appointment
Prior to the scheduled appointment, the following information must be faxed or mailed to the Memory Clinic Nurse:
- Medical records pertinent to diagnosis.
- The completed Request for Consultation (PDF) form.
