Please be prepared to provide your current insurance card at the time of your appointment to determine your payment obligation.
HMO's and PPO'sHMO and PPO patients are expected to obtain a referral for the visit (if required by your plan) and to pay the co-payment specified by the plan at the time of service. Please familiarize yourself with your specific coverage to understand your payment obligation. Contact your insurance company to determine your plan's specific requirements and coverage options.
MEDICAREMedicare patients are not required to make payment at the time of service. Any deductible or patient responsibility will be determined after billing Medicare.
MEDICAIDMedicaid patients are required to bring their current Medical Assistance card reflecting coverage eligibility for the appointment date. If your plan requires a co-payment, it will be due at the time of service. If you do not have your card, we may not be able to verify your plan eligibility and you will be considered a self-pay patient.
COMMERCIALWe accept and bill commercial plans. Please familiarize yourself with your specific coverage to understand your payment obligation.
SELF PAYWe offer a 20% prompt-pay discount to patients paying their estimated charges at the time of service. Our financial counselors will work with the patient and care provider to determine an estimate for the costs of the services provided for the visit. However, this is only an estimate and any remaining balance will be billed to the patient. We accept cash, personal checks, money orders, Visa, MasterCard, Discover and American Express. Patients who are unable to meet the payment expectations outlined above may contact our office to discuss alternative arrangements (see Community Care program below).
