University of Minnesota Physicians has made it easy for you to refer patients to any of our physicians through our Physician Referral Service. Through this service you can schedule patient appointments or request consultations with our specialists.

Referring Physicians and their staff now have three options to submit a request for consultation.
1) Call the number below and speak to a representative of our Physician Referral Service.
2) Download a printable request form and fax the completed form to us at 612-884-0659.
3) Submit an online referral request. See note below regarding network access to the referral form.

Appointments
Patients with urgent medical or surgical needs are given priority in the University of Minnesota Physicians appointment system. Appointments for non-urgent problems are scheduled several days to several weeks in advance.

University of Minnesota Physicians will, to the fullest extent of its resources, accept emergency cases on the same or next day. Patients requiring hospitalization may be referred to an emergency room for evaluation.

Information Needed to Schedule the Appointment
Please be prepared to provide the following information when you call for an appointment:

Patient's Information (name, gender, address, phone number, date of birth)
Reason for the referral, symptoms and diagnosis (including services you would like University of Minnesota Physicians to provide)
Referring Physician Information (UPIN/NPI, physician name, clinic, phone and fax number, address, contact name)

HMO and PPO
HMO and PPO patients are expected to obtain a referral for the visit (if required by their plan) and to pay the co-payment specified by the plan at the time of service. Please have your patient familiarize themselves with their specific coverage to understand their payment obligation. They should contact their insurance company to determine their plan's specific requirements and coverage options.

Medicaid
University of Minnesota Physicians participates with the state Medicaid programs of Iowa, Minnesota, Montana, North Dakota, South Dakota and Wisconsin. If your patient is from one of these states and has out-of-state prior authorization restrictions, the patient should contact the appropriate people to complete these requirements. Except in emergency situations, University of Minnesota Physicians will not provide care to Medicaid patients from other states unless arrangements are made in advance to fully pay billed charges for both the physicians and hospital services.

Medicaid patients are required to bring their current Medical Assistance card. If their plan requires a co-payment, it will be due at the time of service. If the patient does not have your card, we may not be able to verify plan eligibility and the patient will be considered self-paying.

Patient Follow-Up
University of Minnesota Physicians will provide a summary of diagnostic reports, treatments and follow-up care to the referring physician. Please contact our Physician Referral Service if you have any questions or wish to discuss patient care.

Many special services are available for patients. If your patient has a unique need or would like a full listing of services, the patient may visit our web site at www.umphysicians.umn.edu or call us at 612-672-7000 for adults and 888-KIDS-UMN for children.

For technical reasons related to the University and Fairview computer networks, certain users may be unable to access the secure form using the standard address. To submit a Request for Consultation from within the University of Minnesota Campus or from a Fairview facility, please use this alternate address to access the form. We apologize for any inconvenience this may cause and we welcome your referral requests.
